    I am an adoption support specialist with one of the new federal Regional Extension Centers (RECs) for Health Information Technology implementation under provisions the ARRA / HITECH Act. I found this book very illuminating, have cited it on my independent REC blog, and have passed it around our office. The cut-to-the-chase core take-away from this very worthy book is pretty straightforward:

    - The truly difficult work begins after EHR go-live implementation. This is NOT about "IT" per se;

    - You simply must have HIT physician champions;

    - And, you must devote extended post go-live resources to function-based training, delivered via simulators and Adult Learner modeled content -- i.e., the typical constrained, didactic, classroom-based (and "train-the-trainer") methods simply do not suffice.

    Anything less is a recipe for failure (regarding which they enumerate some painfully honest examples from within their own organization).

    This book is particularly instructive for HIT implementations within larger, more complex clinical settings such as multidisciplinary outpatient group practices and inpatient settings. A serious danger in such organizations is permitting IT departments to "own" (and drive) the implementation, adoption, and administrative support processes. You risk having the EHR end-users to become to be viewed as yet one more cohort of always-complaining IT users, for whom "fill out a help desk ticket" can become a standard response. EHRs, however, are quite akin to (and increasingly becoming viewed in the medical liability arena as) de facto medical devices, and, as such their optimal functioning is vital for patient safety and quality of care as well as the bottom line ROI value-add.

    A highly recommended, relatively compact (and inexpensive) read, one replete with relevant, targeted metrics along with implementation lessons-learned case studies and theory.

    This is a great text and I recommend it to anyone who works in the informatics/EHR/healthcare IT arena. I just bought 20 copies for all our deployment staff. Its a quick read and provides a wealth of information regarding post EMR deployment strategies. Strongly recommended.
